DISCHARGE PLANNING
When you are ready to leave Canopy Cove, our treatment team helps you develop a comprehensive treatment plan before you leave.  If you worked in therapy prior to Canopy Cove, we help you transition back to your previous therapist.  We also provide you with a summary of treatment outcomes and our recommendations for on-going therapy and continuity of care.  If you do not have a therapist, we can assist you in locating someone in your area prior to your leaving the program.

 

OUT-PATIENT CARE
Following your treatment at Canopy Cove, after care on an outpatient basis is provided through Health Management Institute (HMI) and is located on our property.  To maintain continuity, your care through HMI will be provided by some of the members of our treatment team who you received services from during your residential or partial hospitalization stay.  We carefully assess your progress and make adjustments as needed.  Continued care is available for as long as you need it.
